Is eligible for the CT Home Care Program for Elders or PCA waiver. CHCPE Income limits for the client only (they need to meet Medicaid income requirements for Category 3 and state income requirements for Category 2)

What is Assisted Living?
Assisted Living is …“a special combination of housing, supportive services, personalized assistance and health care designed to respond to the individual needs of those who need help with activities of daily living and instrumental activities of daily living. Supportive services are available 24 hours a day to meet scheduled needs in a way that promotes maximum dignity and independence for each resident and involves the resident’s family, neighbors, and friends” (Associated Living Federation of America).
Assisted living bridges the gap between independent living and nursing homes. Assisted living is designed for people who want to live in a community setting and who need help with activities of daily living (ADLs), but who do not need as much care as they would receive at a nursing home.
Who is eligible?
Candidates for the Assisted Living Program must meet all CHCPE eligibility requirements met in addition to housing facility requirements. Eligibility requirements include functional needs (assistance with activities of daily living) and financial eligibility criteria.
